Care Manager

A Renewed Mind is a private, not for profit 501(c)(3) behavioral health care organization. We work to deliver personalized, high quality behavioral health services to our community in a compassionate manner. We have developed a reputation of providing quality services based on our commitment to respecting the individual and in forming strong therapeutic relationships.
We are currently looking for qualified individuals to fill a full-time Care Manager position in Toledo, Ohio. Candidates must have, at minimum, a high school diploma, 3+ years of relevant experience and a valid driver's license with good driving record.

Summary
This is an hourly position with hours and schedule established to complete the required duties. This position is not exempt from wage and hour regulations as defined by the U.S. Department of Labor and is therefore granted protections under the Wage and Hour Division of the Fair Labor Standards Act. 

EDUCATION/EXPERIENCE/LICENSE/CERTIFICATION
Required:
  • High school diploma
Three years of relevant experience

  • Valid driver's license and good driving record
Preferred
Bachelor's degree in a related field
  • QMHS or CDCA

ESSENTIAL DUTIES AND RESPONSIBILITIES
  1. Monitor the needs of persons served’s mental illness or substance use and physical symptoms as well as their response to treatment; communicate these changes to the treatment team to ensure immediate and appropriate interventions are provided.
  2. Work with families of persons served to identify needs and connect them with resources to meet those needs.
  3. Coordinate community resources including school, probation officers, referral sources and outpatient mental health providers to ensure continuity of care.
  4. Facilitate appointments to ensure overall physical well-being including basic, mental health and substance use needs.
  5. Achieve productivity goals as assigned.
  6. Perform duties in accordance with applicable professional ethics and standards and established A Renewed Mind policies.
  7. Comply with all state and federal laws regarding protected health information.

ADDITIONAL DUTIES AND RESPONSIBILITIES
  1. Perform job duties in a manner that’s friendly, considerate, and aligned with the agency’s Mission, Vision, and Standards of Ethical Conduct.
  2. Organize and plan work for maximum efficiency and effectiveness.
  3. Utilize IT system and software programs in accordance with agency policy.
  4. Utilize time off in a manner consistent with agency policy.
  5. Perform other duties as deemed necessary and appropriate by supervisor.

ESSENTIAL KNOWLEDGE, SKILLS, ABILITIES, AND PERSONAL CHARACTERISTICS
  1. Ability to learn and utilize the agency’s Electronic Health Record database and other electronic tools effectively and efficiently within 90 days of hire.
  2. Demonstrate a current knowledge of mental health practices.
  3. Demonstrate strong interpersonal and customer service skills.
  4. Demonstrate knowledge of community resources and partnerships available for persons served.
  5. Demonstrate excellent communication skills, including above average written communication; previous training or educational experience is valued.
  6. Demonstrate the ability to work independently.
  7. Demonstrate the ability to effectively function as an A Renewed Mind team member.
  8. Demonstrate the ability to effectively manage multiple tasks concurrently.
  9. Possess and demonstrate effective organizational skills.
  10. Demonstrate the ability to customarily and regularly exercise discretion and good professional judgment. 

PHYSICAL DEMANDS

  3. Ability to perform job responsibilities in facilities with multiple levels and without elevator or mechanical transportation.
